The report is made out to the teacher and school so please leave the details required. The 'report date' can span the time that the teacher has had your child in their class - for example Sep 2018 - Jul 2019. We also need something that they are known for - for example their jazzy skirts, patterned ties etc. to feature on the report. We also need your childs name for the reporting officer and a short personalised message from the child.
